What is the UFB Food Box Program?

Utah Food Bank provides free, monthly food assistance to homebound people with disabilities living in poverty who are over the age of 18, delivered right to the client’s home. The primary goal of this program is to provide emergency food assistance for individuals who cannot access the emergency food network and are not participating in the Commodity Supplemental Food Program (CSFP).

Each food box contains approximately one week’s worth of nonperishable food, plus as much fresh food as available. Last year, 7,345 of these food boxes were delivered—primarily by caring volunteers—who were often some of the few visitors these clients received.

Eligibility Requirements

  • Reside in the following counties: Salt Lake, Washington, Beaver, Iron, Kane or Garfield
  • Be 18 years of age or older
  • Have income at or below 185% of federal poverty guidelines
  • Be homebound due to disability
  • Must be ineligible for CSFP (60+ y.o., income below 150% FPL)
  • Must not live where meals are provided

Medical Authority Note

Applicants may need to submit a note indicating their homebound condition. The Medical Authority Note will be provided during the in-home sign-up appointment.

What happens if I miss receiving my box?

If you miss two months of delivery without notifying Utah Food Bank, your food box deliveries will be discontinued, and you will need to re-apply. If you are in a hospital, out of town, or unavailable for delivery due to illness for two (2) consecutive months, you may remain on the program and will not be removed, if you contact Utah Food Bank at 801.887.1271.

Do I have to reapply every year?

Every year, Utah Food Bank staff will contact you to verify your eligibility for the program. You will receive a call every year and an in-home visit every 3 years. We will ask about your updated income and if there are any changes in your contact information. Because this program is for those with disabilities that cause them to be homebound, we may verify your homebound status again.

Who do I talk to if my information changes?

Your current contact information is essential to the program. We cannot deliver to you if we don’t have your correct number or address. Please call us at 801.887.1271 if any of your contact information changes, such as phone number and address.

Is there an age requirement?

Clients must be at least 18 years old. Those who are age 60 or older may be transferred to CSFP, a food box program for seniors. Commodity Supplemental Food Program (CSFP) is a federally funded program for seniors. As per Utah Food Bank policy, clients 60 years and older must be assessed for eligibility for CSFP. Prior to your 60th birthday, we will inquire whether your income meets CSFP requirements and send you an application to complete. If eligible, you will be switched to CSFP, which is also a monthly food box. If your income is above CSFP requirements but still within UFB requirements, you will continue to receive the UFB food box.
